Attendant Care Package (Disability):

Attendant Care Packages (ACP) provides assistance to individual in carrying out those activities of daily living which, because of functional limitations,individuals are unable to adequately perform themselves. Under the Disability Services Act this support could include assistance with mobility,Siloutte of two people transfers, personal hygiene, bathing, grooming, dressing, eating, drinking and similar personal activities. The maximum amount of care that can be provided under the Program is 34 hours per week.

Assistance is provided by the carer on a “drop-in, not ''live-in'' basis. The assistance may be provided by one carer, or by several carers on a roster basis. Carers are employed by the service provider, according to the needs of the consumer/s, on a full-time, part-time or sessional basis.

Assistance is primarily provided within the home of the client. ACP support may include assistance with basic self care tasks, such as personal hygiene, dressing, grooming, meal preparation, eating and drinking, or use and care of assisting devices or communication.

Am I Eligible?

To be eligible for a Hunter Integrated Care Inc Attendant Care Program the person needs to:

  • Live in a nursing home, hostel or long-term hospital bed or living at home, and residence in a nursing home, hostel or hospital as a nursing home type patient is their only long-term living option.

  • Be aged between 16 and 65 years of age.

  • Have a physical disability requiring up to 34 hours per week of attendant care.

  • Have health/medical problems that can be managed in the community.

  • Assessed and approved by Department of Ageing, Disability & Homecare for a ACP.
