Community Aged Care Packages: (Aged Care)

This service provides a select range of services to older persons who normally reside in the Hunter Valley, to enable them to remain at home. Services which may make up the “package” of services for a particular client include:

Cleaners

Personal Care

Housework

 
Laundry

Laundry

Meal Preparation

Meals on Wheels

Shopping

Transport

Monitoring of Medications


The Aged Care Assessment Team (ACAT) must assess all potential recipients of a package. ACAT will determine your suitability to receive care in your home as an alternative to admission to hostel care. A Case Manager will visit you at home to discuss your needs and negotiate the services to be provided. The care plan is developed with the client and significant others.

Am I Eligible?

To be eligible you should:

  • Resides in the Dungog, Maitland, Muswellbrook, Singleton or Upper Hunter local government areas, and

  • Able to weight bear and transfer independently and

  • not reside in a hospital, group home or residential care facility, and

  • be assessed by the ACAT to receive a community care package.

Output:

60 packages—minimum of 5 in Dungog, Maitland, Murrurundi, Muswellbrook, Scone and Singleton Shires. 8 packages in Maitland.
