Hawks Nest / Tea Gardens Respite Options (HACC)

Respite Options provides a flexible and innovative  range of  services to meet the respite needs of carers of frailPeople with dog, older person or a person with a disability, aged to 65 years.
Once the clients’ needs are identified a Respite Care Plan will be  developed that will allocate a fixed financial amount to a client for respite for a specified period, usually 6 or 12 months.
There is no guarantee that funds will be available for another respite package at the conclusion of each period

The Case Manager will discuss with the carer the options for receiving the care. The carer may choose, within the allocated resources, the days and times that respite occurs.

Am I Eligible?

  • Be the carer of a person who is frail, aged or a younger persons with a disability, and
  • not be living in a hospital, group home or residential care facility, and
  • not be reTo be eligible you should:ceiving care under a Community Aged Care Package, Service Access System or Attendant Care program, and
  • reside in the rural areas of the Great Lakes Local Government area.

Outputs:

1,700 hours, of Respite per annum.
