Lower Hunter Dementia Community Options (HACC):

This service is designed to provide intensive support and case management of HACC clients with a diagnosis of dementia, who have complex needs and require ongoing monitoring. This service is provided to assist clients to remain at home rather than go into residential care. Eligible clients are assessed and if Hunter Integrated Care Inc. has the resources to provide the required services, a care plan is developed, implemented and monitored by a Case Manager. The care plan is developed with the client and it is important that a family member or friend is present when the services and care plan are discussed.

Am I Eligible?

To be eligible you should:

• Have a diagnoses of Dementia, and
• have complex needs, and
• not reside in a group home or residential care facility, and
• not be receiving care under a Community Care Packages, ServiceAccess System or Attendant Care program, and

• reside in the Dungog, Maitland or Port Stephens local government areas.
