Upper Hunter Community Options (HACC):

This program is designed to provide intensive support and case management of HACC clients who have complex needs and require ongoing monitoring. This service is provided to assist  clients to remain at home rather than go into residential care.
Eligible clients are assessed and if Hunter Integrated Care Inc. has the resources to provide the required services,  a care plan is developed, implemented and monitored by a Case Manager.
The care plan is developed with the client and it is important that a family member or friend is present when the services and care plan are discussed.

Am I Eligible?

To be eligible you should:
  • Be a person who is frail, older person or person with a disability, and
  • have complex needs, and
  • not be living in a hospital, group home or residential care facility, and
  • not be receiving care under a Community Care Packages, Service Access System or Attendant Care Program, and
  • resides in the Muswellbrook, Singleton or Upper Hunter local government areas.
