Hunter Integrated Care Inc. is in a consortium with Novacare Inc. to provide Veteran’s Home Care across the 9 local government areas in the Hunter. Hunter Integrated Care Inc. provides services to veterans living in Dungog, Muswellbrook, Singleton and Upper Hunter Shires. The identification of your requirements and the resultant plan of care, outlining your needs has to be performed by the DVA Assessment Agency, 1300 550 450, who will work with the consortium to ensure that this plan meets your changing needs. Hunter Integrated Care Inc & Novacare can only provide service to you after the DVA Assessment Agency has issued a care plan detailing the service you will receive and co-payment you are required to pay. Once we have received the Care Plan, Hunter Integrated Care Inc. will endeavour to plan your care for the day and time which is most convenient for you, however, some flexibility may be needed due to staffing constraints. Services are not normally given on public holidays unless by prior arrangement with your Case Manager but services can be arranged for an alternative day at a mutually convenient time. |
